Need help determining whether your insurance covers nutrition counseling or diabetes education? Call the member services number on the back of your insurance card and ask for the following information:

Does my plan cover outpatient [nutrition counseling] or [diabetes education]? (You can give the representative the CPT Code 97802 for nutrition counseling with diagnosis code Z71.3 or G0108 for diabetes education with diagnosis code E11.9 (Type 2) or E10.9 (Type 1). If yes, how many visits are allowed?

Do I need a physician referral? (If you need a physician referral, it must be done prior to your first visit. You will need to call your physician’s office and have them fax over the referral to our office (512-737-3756).

Do I have a deductible to meet before my visits are covered? If yes, how much?

Will I have a copay or coinsurance? If yes, how much?
